When you book a consultation, our team will request your insurance information in advance. This allows us to confirm whether your plan is likely to cover care at GAMMA—both the procedure you’re seeking and the surgeons providing it—before your visit.

After your consultation, you’ll meet with our insurance review and scheduling specialist. This dedicated team member will help clarify your plan’s cost-sharing structure, identify any missing requirements, and walk you through the next steps, including prior authorization and timeline planning.

What We Need From You

To begin the process, please provide the following:

• Insurance Card
  A photo or scan of the front and back of your current insurance card.
• Multiple Policies
  If you have more than one insurance policy, let us know which plan is designated as primary. (If you’re unsure, your insurance providers can clarify.)
• Insurance Updates
  Let us know as soon as possible if your insurance coverage changes.
